Back to the date from 17th
century the Latin phrase Primum
Non Nocere (PNN)—“First Do No Harm”— carried over the
years with the principle of ethics taught to its disciples, i.e. “in an
existing problem, it may be better not to do something, or to do nothing, than
to risk causing more harm than good”. It is such a splendid way to presage
a healthcare provider to seek Primum Non Nocere when
considering use of any intervention or investigation that holds either no
benefit or at times carries potential risk.
But sadly this has become a mere formality to
physicians to practice Primum Non Nocere. On contrary opponent
physicians follow over-utilization or unnecessary medical treatment or
investigations, they often advice unnecessary medications that push the patient
into a trench of many diseases. The medicines like antibiotics, analgesics etc
are generally misused arbitrarily in every case. Say a case of acute
sinusitis or flu etc. may not require antibiotics even then patient
is loaded with antibiotics or NSAIDs without second
thought of First Do No Harm that these medicines carry bunch
of side effects. This malpractice is not just demon to
the patient but to the society as a whole.
It is very unfortunate to
hear that owing to injudicious execution of treatment procedures by un-sighted
physician may land the patient into the swamp of diseases and also surrender
body to the obnoxious virus or bacteria. An unnecessary medical
intervention of a self-limiting condition or that requires only limited
treatment may culminate the disease into more damaging one (Physician
Induced Iatrogenic Disease), disease which is far more difficult to
uproot than the natural one.
The advanced technology of modern medicine
though has conquered many deadliest life taking diseases (mortality) in the
past but it has also invited more destruction and injuries to the patients
(morbidity) by injudicious unnecessary medical treatment ("over-utilization,"
healthcare provided with a higher volume or cost than is appropriate) resulting
from a physician's decision, medical errors (inaccurate or
incomplete diagnosis or treatment of a disease, injury, infection, or other
ailment), negligence (failure to exercise appropriate and or
ethical ruled care expected to be exercised amongst specified
circumstances), use of contaminated instruments, and last but not
the least complications arising from a medical procedure or treatment.
A study released in 2016 found medical error is
the third leading cause of death in the United States, after heart disease and
cancer. It is extrapolated that over 250,000 deaths (9.5%) per year were
consequence of medical error in the US, analysis of medical death rate from
2000 to 2008 (Frellick,
Marcia, 3 May 2016).
Human error is inevitable, but still it is in
our hand, we can control it by endeavoring Primum Non Nocere and providing
a safe treatment without side effects, while dodging unnecessary
medical treatments, negligence, medical errors to
its patients and do justice to this noble profession.
We have a responsibility towards the patient and
we have to be idealistic in the application of our professional standards our
prime directive, Primum Non Nocere. First and foremost, if we can’t
help, we must never harm a patient.

responsibility of all healthcare system”.
